ABSTRACT

Abstract The aim of this study was to assess the centralization and dentin thickness of mesial root canals of the first mandibular molars by microcomputed tomography (micro-CT). Material and methods: Ninety-nine mandibular molars of Vertucci's type IV canals were scanned by micro-CT. The mesiodistal deviation and centroid were assessed, in both mesiobuccal (MB) and mesiolingual (ML) canals, for the apical 4mm and the full canal length. Results: The dentin thickness was similar for both MB and ML canals. The narrowest thickness was in the distal wall of an MB canal (0.07mm), while the widest was found in the mesial wall of an MB canal (2.46mm). In centroid analysis, both the MB and ML canals exhibited deviations when compared to the root centroid, along the full canal length and the apical 4mm. For the MB canal, the mean deviation was 0.83mm (0.02 mm-2.30 mm) for the full canal and 0.18mm (0.01 mm-1.01 mm) for apical 4mm. Similarly, for the ML canal, the mean deviation measured 0.83 mm (0.05mm-3.99mm) for the full canal and 0.21 mm (0.01mm-1.01mm) for the apical 4 mm. Overall, deviations were observed towards the mesial of the roots, with 69% for MB and 57% for ML canals for the full canal, and 51% for MB canals within the 4 mm. The exception was the ML canal, which exhibited a higher deviation towards distal in the apical 4mm, accounting for 52% of cases. The dentin thickness was consistent between the mesial canals of mandibular molars. However, there is no centrality of mesial canals in their roots, with frequent deviation to mesial.


Resumo O objetivo deste trabalho foi avaliar a centralização e a espessura da dentina dos canais radiculares mesiais de primeiros molares inferiores por meio de microtomografia computadorizada (micro-CT). Material e métodos: Noventa e nove molares inferiores com canais tipo IV de Vertucci foram escaneados por micro-TC. O desvio mesiodistal e o centroide foram avaliados para os canais mesiovestibular (MB) e mesiolingual (ML), nos 4mm apicais e em todo o comprimento do canal. Resultados: A espessura da dentina foi semelhante para os canais MB e ML. A espessura mais estreita foi encontrada na parede distal de um canal MB (0,07mm), enquanto a mais larga foi encontrada na parede mesial de um canal MB (2,46mm). Na análise centroide, tanto o canal MB quanto o ML exibiram desvios quando comparados ao centroide da raiz, ao longo de todo o comprimento do canal e nos 4 mm apicais. Para o canal MB, o desvio médio foi de 0,83mm (0,02mm-2,30mm) para canal inteiro e 0,18mm (0,01mm-1,01mm) para o apical de 4mm. Da mesma forma, para o canal ML, o desvio médio mediu 0,83 mm (0,05 mm-3,99 mm) para o canal inteiro e 0,21 mm (0,01 mm-1,01 mm) para os 4 mm apicais. No geral, foram observados desvios em direção mesial das raízes, sendo 69% para canais MB e 57% para canais ML para canal inteiro, e 51% para canais MB dentro dos 4 mm. A exceção foi o canal ML, que apresentou maior desvio para distal nos 4mm apicais, representando 52% dos casos. A espessura da dentina foi consistente entre os canais mesiais dos molares inferiores. Entretanto, não há centralidade dos canais mesiais em suas raízes, com frequente desvio para mesial.

ABSTRACT

Abstract This study compared the preparation of long oval-shaped root canals using WaveOne Gold and XP-endo Shaper systems, both supplemented or not with manual instrumentation. Twenty-four long oval-shaped canals of mandibular incisors were distributed into two groups according to the instrumentation: WaveOne Gold Primary or XP-endo Shaper systems. All root canals were manually instrumented with a size 25 K-file after automated preparation. The specimens were scanned by using a micro-CT device (17.42 µm) before and after automated preparation and manual instrumentation. The increased surface of the root canal and the percentage of untouched areas were assessed. Both WaveOne Gold and XP-endo Shaper systems increased the root canal surface and had similar untouched areas (p>0.05). Supplementary instrumentation increased the surface of the root canal and decreased the untouched walls (p<0.05). WaveOne Gold and XP-endo Shaper systems provided a similar preparation of long oval-shaped canals and manual instrumentation improved their preparation.


Resumo Este estudo comparou o preparo de canais radiculares ovais longos usando os sistemas WaveOne Gold e XP-endo Shaper, ambos complementados ou não com instrumentação manual. Vinte e quatro canais longos e ovais de incisivos inferiores foram distribuídos em dois grupos de acordo com a instrumentação: sistemas WaveOne Gold Primary ou XP-endo Shaper. Todos os canais foram instrumentados manualmente com uma lima K tamanho 25 após preparo automatizado. Os espécimes foram escaneados usando um dispositivo de micro-CT (17,42 µm) antes e após o preparo automatizado e instrumentação manual. O aumento de superfície do canal radicular e a porcentagem de áreas intocadas foram avaliadas. Ambos os sistemas WaveOne Gold e XP-endo Shaper aumentaram a superfície do canal radicular e apresentaram áreas intocadas semelhantes (p>0,05). A instrumentação complementar aumentou a superfície do canal radicular e diminuiu o percentual de áreas intocadas (p <0,05). Os sistemas WaveOne Gold e XP-endo Shaper tiveram o preparo de canais ovais longos semelhantes, e a instrumentação manual melhorou o preparo.

ABSTRACT

Objective:To study the effect of Bushen Huatan prescription on helper T cell 17 (Th17)/T regulatory cells (Treg) balance of immune T cell subsets in the prevention and treatment of postmenopausal osteoporosis. Method:Sixty 6-month-old female SD rats were randomly divided into sham operation group, model group, estradiol valerate group (0.184 mg·kg<sup>-1</sup>) and Bushen Huatan prescription low, medium and high groups (4.7, 9.4, 18.8 g·kg<sup>-1</sup>) according to the random number table. All the groups except the sham operation group received ovariectomy to make postmenopausal osteoporosis model. Intragastric administration was started 1 week after operation, and the rats in model group and sham operation group received equal volume of normal saline, once a day for 12 weeks. Microcomputed tomography (Micro CT) was then used to detect bone mass and microstructure of rats, the contents of Forkhead box protein (Foxp3) and retinoic acid related nuclear orphan receptor (ROR<italic>γ</italic>t) in serum were detected by enzyme-linked immunosorbent assay (ELISA), the mRNA expression levels of Foxp3 and ROR<italic>γ</italic>t in bone tissues were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot was used to detect the protein expression of Foxp3 and ROR<italic>γ</italic>t in bone tissues, the number of Th17 and Treg cells in each group was analyzed and compared by flow cytometry. Result:Compared with the sham operation group, the bone mass and trabeculae of the model group decreased (<italic>P</italic><0.01), the bone microstructure was destroyed, the concentration of Foxp3 in serum decreased, the concentration of ROR<italic>γ</italic>t increased (<italic>P</italic><0.01), the mRNA and protein expression levels of Foxp3 in bone tissues decreased, ROR<italic>γ</italic>t increased, the number of Treg cells in bone tissues decreased, number of Th17 cells increased (<italic>P</italic><0.01), and Th17/Treg ratio increased (<italic>P</italic><0.01) in model group. Compared with the model group, the bone mass in each treatment group increased (<italic>P</italic><0.05, <italic>P</italic><0.01), Foxp3 concentration in serum increased, ROR<italic>γ</italic>t concentration decreased (<italic>P</italic><0.01), the mRNA and protein expression levels of Foxp3 in bone tissues increased significantly (<italic>P</italic><0.05, <italic>P</italic><0.01), but no statistical difference was shown in mRNA expression between low dose group and the model group. In addition, the mRNA and protein expression of ROR<italic>γ</italic>t decreased (<italic>P</italic><0.05, <italic>P</italic><0.01), number of Treg cells increased, number of Th17 cells decreased (<italic>P</italic><0.05, <italic>P</italic><0.01), and Th17/Treg ratio decreased in treatment groups (<italic>P</italic><0.01). Conclusion:Bushen Huatan prescription can increase bone mass, improve bone microstructure, increase the number of Treg cells and decrease the number of Th17 cells in ovariectomized rats. It is concluded that Bushen Huatan prescription may play a role in preventing and treating postmenopausal osteoporosis by regulating Th17/Treg balance.

ABSTRACT

Abstract Objectives This study assessed the incidence and variability features of root canals system (RCS) and their ramifications according to Pucci & Reig (PR) (1944) and the American Association of Endodontists (AAE) (2017) by micro-computed tomography (μCT). Methodology 500 representative extracted human teeth of each tooth group (n=50) (maxillary/mandibular central and lateral incisors, canines, first and second premolars and molars) were scanned by μCT with a resolution of 26.70 μm. The reconstructed cross-sections images and the visualization of the continuous slices in the transversal axis were performed using DataViewer software. RCS were classified according to Pucci & Reig (main canal, collateral canal, lateral canal, secondary canal, accessory canal, intercanal, recurrent canal) and AAE (main canal, accessory canal, lateral canal). The apical deltas were assessed for both classifications. The prevalence of apical deltas was evaluated using the Chi-squared test (p<0.05). Results According to PR, a higher incidence of lateral canals was observed in maxillary canines (10%), central incisors (8%) and first premolars (6%). Using AAE, the highest incidence of lateral canals was observed in the mandibular first premolars (85%), first and second molars (84%), lateral incisors (67%), canines (59%), and in maxillary first premolars (52%). Regarding accessory canals, the PR showed a frequency in 2% of the maxillary lateral incisors and maxillary and mandibular first premolars and 3% of mandibular first and second molars. On the other hand, the AAE showed the highest incidence of accessory canals in 86% of the maxillary first premolars, 71% in mandibular lateral incisors, 69% in mandibular first premolars, 65% in mandibular canines, and 56% in maxillary canines. The PR showed the lowest incidence of apical deltas for all dental groups when compared with AAE (p=0.004). Interestingly, distal canals in maxillary molars showed a significant discrepancy between classifications (p=0.027). Conclusions μCT enabled accurately describing the RC system and related ramifications, adding to the PR and AAE classifications, with some discrepancies reported for maxillary molars. Clinical Relevance This μCT study enabled a thorough description of the variability among root canals and their ramifications, including clinically relevant details on the presence and location of lateral canals and accessories in all human tooth groups, beyond the currently existing classification systems.

Abstract Isthmuses are reported as common anatomic complexities in teeth often associated with failures in endodontic treatment. They should be considered before starting treatment and a preoperative computed tomography scan (CT) may demonstrate these complexities. Objective To assess the diagnostic value of the highest resolution settings of a cone-beam CT (CBCT) system in identifying and measuring apical isthmuses, using micro-CT as reference. Methodology After micro-CT scanning, 40 humans' lower first molars with isthmuses in the apical-3 mm of mesial roots were scanned by the highest resolution settings of the New Generation i-Cat ® CBCT equipment. Two blinded observers recorded the detection of isthmuses in CBCT scans. The lengths of isthmuses were compared between micro-CT and CBCT to assess the diagnostic value of CBCT. Quantitative data for sensitivity were represented as percentages (95% confidence interval). The Bland-Altman method was used to assess differences between gold standard lengths (micro-CT) and CBCT lengths. Results BCT demonstrated 30 positive findings, representing sensitivity for isthmus identification of 75% (95% CI=0.4114-1.1364). Differences between the lengths in micro-CT (1.99±0.40 mm) and CBCT (1.53±0.41 mm) were significant (p<0.0001). Conclusion The CBCT device used presented limited diagnostic value in the identification and measurement of apical isthmuses in the mesial roots of lower molars. In some cases, the actual anatomy of the apical root canal may not be completely delineated in this type of CBCT system, even using the highest resolution settings.

ABSTRACT

Objetivo: Comparar la presencia de poros en los tres tercios del conducto radicular luego de la obturación con cementos biocerámicos. Se trataron endodónticamente 20 premolares inferiores unirradiculares, de anatomía oval. Los mismos fueron divididos en dos grupos y se obturaron con dos cementos biocerámicos diferentes. Todas las muestras fueron analizadas con microtomografía de rayos X para comparar la presencia de poros en los tres tercios radiculares, clasificando los mismos en internos, externos y combinados. En las 20 piezas dentarias obturadas y analizadas se encontraron poros. La cantidad de poros detectados no presentó diferencias significativas mediante análisis estadísticos cuantitativos ni cualitativos. Los poros se presentaron más frecuentemente en el tercio cervical, independientemente del cemento sellador. Ambos grupos presentan una buena adaptación a nivel apical, siendo esto imprescindible para la longevidad y éxito del tratamiento endodóntico (AU)

ABSTRACT

Este estudo teve como objetivo avaliar, por meio de microtomografia computadorizada, a eficiência do instrumento XP-3D Shaper (XPS) no preparo de canais distais ovais de molares inferiores utilizando o sistema ProTaper Next (PTN) como grupo controle, e comparar o melhor resultado aos sistemas TRUShape (TRS) e WaveOne Gold (WOG). Raízes distais moderadamente curvas (10°-20°) de molares inferiores com canais ovais únicos foram selecionadas e escaneadas a 21µm de tamanho nominal de voxel. Na fase 1, doze espécimes foram aleatoriamente distribuídos em dois grupos: XPS (grupo experimental) e PTN (grupo controle). Após os escaneamentos iniciais, cada amostra alocada no grupo XPS foi escaneada três vezes subsequentes após instrumentação com 5 movimentos até o comprimento de trabalho (CT) (0,5 mm) (subgrupo XPS5); após 10 movimentos adicionais (subgrupo XPS15); e após outros 10 movimentos adicionais (subgrupo XPS25). As amostras alocadas no grupo PTN foram escaneadas antes da instrumentação e novamente após a preparação final com a lima X3 (30/.07). Na fase 2, trinta espécimes foram aleatoriamente distribuídos em três grupos: XPS, TRS e WOG. Cada amostra foi escaneada antes e após a instrumentação até o CT (0,5 mm). Alterações nos parâmetros morfológicos, porcentagem de áreas não tocadas e o tempo de preparo (em segundos) foram avaliados. A análise estatística foi realizada (α = 5%). Na fase 1, em relação às áreas não tocadas (%), o XPS5 (9,02 ± 5,90) não diferiu significativamente do PTN (16,85 ± 7,75) (p > 0,05). XPS15 (4,13 ± 3,38) e XPS25 (2,35 ± 2,03) apresentaram significativamente menor porcentagem de área não tocada em comparação com XPS5 e PTN (p < 0,05). A diferença entre XPS15 e XPS25 não foi significativa (p > 0,05). O tempo médio total de instrumentação foi significativamente diferente entre os grupos PTN (112 ± 9s) e XPS5 (58 ± 7s), e entre PTN e XPS15 (90 ± 7s) (p < 0,05). Na fase 2, o percentual de áreas não tocadas foi significativamente menor com XPS (5,25 ± 2,57) comparado ao TRS (12,45 ± 5,84), e comparado ao WOG (11,46 ± 4,00) (p < 0,05). O XPS removeu significativamente mais dentina (3,29 ± 1,51 mm3) do que TRS (1,89 ± 0,83 mm3) e WOG (1,81 ± 0,80 mm3) (p < 0,05). Em conclusão, o preparo mecânico com XPS utilizando ao menos dez movimentos além do protocolo proposto pelo fabricante melhorou significativamente a capacidade de instrumentação, reduzindo a porcentagem de área não tocada. O XPS removeu mais dentina (mm3) e deixou menos área não tocada (%) do que TRS e WOG, sem aumentar significativamente o volume do canal (%).


This study aimed to evaluate, using micro-computed tomography, the shaping efficiency of XP-3D Shaper (XPS) in oval canals using ProTaper Next (PTN) system as control group, and compare it to TRUShape (TRS), and to WaveOne Gold (WOG). Moderately curved distal roots (10°-20°) of mandibular molars with single oval canals were selected and scanned at 21µm nominal voxel size. In phase 1, twelve specimens were randomly assorted to instrumentation with XPS (experimental group) and PTN (control group). Following initial scans, each specimen allocated to XPS was scanned three subsequent times: After 5 strokes to the working length (WL) (0.5 mm) (subgroup XPS5); additional 10 strokes (subgroup XPS15); and 10 more additional strokes (subgroup XPS25). PTN samples were scanned prior to instrumentation and again after final preparation to X3 (30/.07). In phase 2, thirty specimens were randomly assorted to instrumentation with XPS, TRS and WOG. Each specimen was scanned before and after preparation to WL (0.5 mm). Changes in morphological parameters, percentage of untouched walls, and preparation time (seconds) were assessed. Statistical analysis was performed (α = 5%). In phase 1, in regard to unprepared areas (%), XPS5 (9.02 ± 5.90) was not significantly different from PTN (16.85 ± 7.75) (p > 0.05). XPS15 (4.13 ± 3.38), and XPS25 (2.35 ± 2.03) had significantly less unprepared areas compared with XPS5 and PTN (p < 0.05). The difference between XPS15 and XPS25 was not significant (p > 0.05). Mean total preparation time was significantly different among the experimental groups, and between PTN (112 ± 9s) to XPS5 (58 ± 7s), and between PTN to XPS15 (90 ± 7s) (p < 0.05). In phase 2, the percentage of unprepared areas was significantly lower with XPS (5.25 ± 2.57) compared to TRS (12,45 ± 5,84), and compared to WOG (11.46 ± 4.00) (p < 0.05). XPS removed significantly more dentin (3.29 ± 1.51 mm3) than TRS (1.89 ± 0.83 mm3), and than WOG (1.81 ± 0.80 mm3) (p < 0.05). In conclusion, preparation using extra ten strokes further than the manufacturer's instructions with XPS significantly improved instrumentation capacity, reducing the percentage of unprepared areas. XPS removes more dentine (mm3) and reduces the unprepared area (%) further than TRS and WOG without significantly increasing the canal volume (%).

OBJECTIVE@#To examine the effects of ursolic acid (UA) on mitigating retinoic acid (RA)-induced osteoporosis in rats.@*METHODS@#Fifty female Sprague-Dawley rats were randomly divided into the control group (n=10) and the osteoporosis group (n=40). The 40 osteoporosis rats were induced by 75 mg/(kg•d) RA once daily for 2 weeks, and then were randomly assigned to vehicle control (model), low-, middle-, and high-dose UA [(UA-L, UA-M, UA-H; 30, 60, 120 mg/(kg•d), respectively] groups (10 rats each). UA were administered once daily to the rats from the 3rd weeks for up to 4 weeks by gavage. Bone turnover markers [serum alkaline phosphatase (ALP), osteocalcin (OCN), urine deoxypyridinoline (DPD)] and other parameters, including serum calcium (S-Ca), serum phosphorus (S-P), urine calcium (U-Ca), urine phosphorus (U-P), and bone mineral density (BMD) of the femur, 4th lumbar vertebra and tibia, bone biomechanical properties and trabecular microarchitecture, were measured.@*RESULTS@#The osteoporosis in rats was successfully induced by RA. Compared with the model group, UA-M and UA-H significantly reversed the RA-induced changes in S-P, U-Ca, U-P, ALP, OCN and urine DPD ratio and markedly enhanced the BMD of right femur, 4th lumbar vertebra and tibia (Plt;0.05 or Plt;0.01). Further, biomechanical test and microcomputed tomography evaluation also showed that UA-H drastically improved biomechanical properties and trabecular microarchitecture (Plt;0.05 or Plt;0.01).@*CONCLUSION@#UA could promote bone formation, increase osteoblastic activity and reduce osteoclastic activity in rats, indicating that UA might be a potential therapeutic of RA-induced acute osteoporosis.

Abstract In order to evaluate the volume of dentinal cracks taking into account the age of the dentin and the type of file system used for endodontic procedures, forty freshly extracted single-rooted lower incisive teeth presenting similar root volume, were divided into two groups according to the age of the patient: Group Young (18 - 30 years old) and Group Old (60 years old or more). Each specimen was scanned by microcomputed tomography (micro-CT) in three stages: (i) before any treatment, (ii) after endodontic treatment with Reciproc files (REC), and (iii) after subsequent endodontic retreatment. Each group was subdivided into two subgroups, according to the retreatment technique used: retreatment with REC or with ProTaper Universal Retreatment (PUR) files. For each subgroup, the images were analyzed for differences in the volume of dentinal cracks in the middle and apical thirds of the teeth , according to the dentin age. In both stages (before and after instrumentation), the micro-CT images of the old root dentin presented with higher volume of cracks than those of the young root dentin, statistical significance notwithstanding (p>0.01). The use of REC files appeared to have no statistically significant differences in the generation of cracks in any type of root dentin when compared with PUR files (p>0.01). When retreated with PUR, the old root dentin presented with a significantly higher volume of cracks (p<0.01) when compared with the old root dentin initially.


Resumo Para avaliar o volume de fissuras dentinárias levando em consideração a idade da dentina e o tipo de sistema de limas utilizadas para procedimentos endodônticos, 40 dentes unirradiculares, incisivos inferiores, apresentando similar volume radicular, recentemente extraídos foram divididos em dois grupos de acordo com a idade do paciente: Grupo Jovem (18 a 30 anos) e Grupo Idoso (60 anos ou mais). Cada espécime foi scaneada por microtomografia computadorizada (micro-CT) em três estágios: (i) antes de qualquer tratamento, (ii) após tratamento endodôntico com limas Reciproc (REC) e (iii) após subsequente retratamento endodôntico. Cada grupo foi subdividido em dois subgrupos, de acordo com a técnica de retratamento utilizada: retratamento com REC ou com lima ProTaper Universal Retreatment (PUR). As imagens foram analisadas quanto às diferenças no volume das fissuras dentinárias nos terços médio e apical dos dentes de acordo com a idade da dentina. Em ambos os estágios (antes e depois da instrumentação), as imagens de micro-CT da dentina radicular idosa apresentaram maior volume de trincas do que as jovens, sem significância estatística (p>0.01). O uso das limas REC mostram não ter diferença estatística significante na geração de fissuras, em qualquer tipo de dentina, quando comparadas às limas PUR (p>0.01). Quando retratada com PUR, a dentina radicular idosa apresentou um volume de trincas significativamente maior (p<0.01) quando comparado com a dentina radicular idosa inicialmente.

ABSTRACT

Abstract Objective: The purpose of this study was to analyze the ability of multiple compounds to seal the dental tubules using scanning electron microscopy (SEM) and micro-computed tomogra-phy (micro-CT). Material and Methods: Twenty-four single-root human mandibular premolars were selected and instrumented with nickel-titanium rotary file and the final file size was #40/06. They were then randomly allocated into 2 groups, and all samples were filled with single cone gutta-percha (#40/06) and one of the tested sealers (AH Plus and EndoSequence BC sealers). All specimens were scanned using micro-CT and then three from each group were randomly selected for SEM analysis. Results: According to SEM, both root canal sealers showed sufficient adaptation to dentin along the whole length of the root canal, though the coronal sections presented superior sealing than the apical sections. Micro porosity analyses revealed that the volume of closed pores and the surface of closed pores had the largest values in the coronal sections, followed by the middle and the apical sections for both sealants (p<0.05). However, no significant difference was observed for those two parameters between AH Plus and EndoSequence BC sealers in any of the three sections (p>0.05), whereas they were larger in the apical section when the AH Plus sealer was used. Conclusions: By using the single cone technique, neither EndoSequence or AH Plus pro-vides a porosity-free root canal filling. The EndoSequence BC sealer may have similar sealing abilities regarding the whole root canal as the AH Plus sealer. A better sealing effect could be obtained in the coronal and middle sections of a root canal than the apical part by using the tested sealers.

ABSTRACT

Beads, a novel drug delivery system self-assembled by cyclodextrins (CDs) and oil, has potential applications in the solidification of oil drugs and improving the bioavailability of lipid-soluble drugs. However, very few researches were dedicated to the mechanism of beads formation. In this study, three-dimensional structures of beads were visualized and investigated using synchrotron radiation X-ray microcomputed tomography (SR-μCT). The structural changes of beads attributed by drying process were analyzed and confirmed via visualization results of SR-μCT. Productively, it was proposed that Pickering emulsion droplets obtained during beads formation process were spatially localized orderly. Moreover, the effects of loading lipid soluble drug, namely, vitamin K1, on the structural changes of beads were also analyzed. It is well known that the surface tension of oil phase could be changed by the addition of lipid soluble constituents. It was reasonable that the three-dimensional structure of beads might be altered during the drug loading of vitamin K1 into the beads. However, although the morphologies of beads were changed to some extent, the ordered Pickering emulsion droplets during the process of beads formation was successfully illustrated based on the SR-μCT results. Conclusively, according to the three-dimensional structural analysis of the beads, this study revealed the organized architecture for Pickering emulsion droplet assembly and beads formation in cyclodextrin semi-inclusion complex, which significantly complements the formation mechanism of beads, and provides a structural basis for the further study of beads.

Abstract The aim of this study was to evaluate morphological features of 70 single-rooted mandibular first premolars with radicular grooves (RG) using micro-CT technology. Teeth were scanned and evaluated regarding the morphology of the roots and root canals as well as length, depth and percentage frequency location of the RG. Volume, surface area and Structure Model Index (SMI) of the canals were measured for the full root length. Two-dimensional parameters and frequency of canal orifices were evaluated at 1, 2, and 3 mm levels from the apical foramen. The number of accessory canals, the dentinal thickness, and cross-sectional appearance of the canal at different root levels were also recorded. Expression of deep grooves was observed in 21.42% of the sample. Mean lengths of root and RG were 13.43 mm and 8.5 mm, respectively, while depth of the RG ranged from 0.75 to 1.13 mm. Mean canal volume, surface area and SMI were 10.78 mm3, 58.51 mm2, and 2.84, respectively. Apical delta was present in 4.35% of the sample and accessory canals were observed mostly at the middle and apical thirds. Two-dimensional parameters indicated an oval-shaped cross-sectional appearance of the root canal with a high percentage frequency of canal divisions (87.15%). Canal configuration type V (58.57%) was the most prevalent. C-shaped configuration was observed in 13 premolars (18.57%), whereas dentinal thickness ranged from 1.0 to 1.31 mm. Radicular grooves in mandibular first premolars was associated with the occurrence of several anatomical complexities, including C-shaped canals and divisions of the main root canal.


Resumo O objetivo deste estudo foi avaliar a morfologia de 70 pré-molares inferiores com depressões radiculares (DR) usando a microtomografia. Os dentes foram escaneados e avaliados quanto à morfologia das raízes e canais radiculares, bem como o comprimento, profundidade, frequência e localização das DR. O volume, a área de superfície e o Structure Model Index (SMI) dos canais foram mensurados no comprimento total da raiz. Parâmetros bidimensionais e orifícios do canal foram avaliados a 1, 2 e 3 mm do forame apical. O número de canais acessórios, a espessura dentinária e a aparência transversal do canal em diferentes níveis de raiz também foram registrados. A expressão de sulcos profundos foi observada em 21,42% da amostra. Os comprimentos médios de raiz e DR foram de 13,43 mm e 8,5 mm, respectivamente, enquanto a profundidade das DR variou de 0,75 a 1,13 mm. O volume médio do canal, a área superficial e o SMI foram de 10,78 mm3, 58,51 mm2 e 2,84, respectivamente. O delta apical estava presente em 4,35% da amostra e os canais acessórios foram observados principalmente nos terços médio e apical. Os parâmetros bidimensionais indicaram canais radiculares com secção oval e alta frequência de divisões canal principal (87,15%). A configuração Tipo V do canal radicular foi a mais prevalente (58,57%). A presença de canais em forma de C foi observada em 13 pré-molares (18,57%), enquanto a espessura dentinária variou de 1,0 a 1,31 mm. Os sulcos radiculares nos primeiros pré-molares inferiores foram associados à ocorrência de várias complexidades anatômicas, incluindo canais em forma de C e múltiplas divisões do canal principal.

ABSTRACT

When endodontic treatment fails, an alternative could be root canal retreatment. During this procedure, all filling material should be removed to allow a new root canal preparation and new obturation of the root canal system. Bacteria are the main cause of endodontic treatment failure, and persistent infection may be related to microorganism ability to penetrate into dentinal tubules. Therefore, this requires the use of irrigating solutions with antimicrobial action and low toxicity. The aim of this study was to evaluate the performance of instruments made of different alloys in root canal re-instrumentation during endodontic retreatment of lateral incisors with apical curvature, using computed microtomography and assessment of these samples by scanning electron microscopy after irrigant activation. Furthermore, the antimicrobial action of a root canal irrigant containing silver nanoparticles, 2% chlorhexidine and 2.5% sodium hypochlorite was evaluated against Enterococcus faecalis biofilm and dentin infected with this microorganism. Thirty extracted maxillary lateral incisors with apical curvature were selected. The teeth were instrumented, filled and divided into three different groups according to the protocol for removal of filling material: Group 1: re-instrumentation with Reciproc R25 instruments, Mtwo 40 and ProDesign Logic 50.01; Group 2: re-instrumentation with ProDesign R, ProDesign Logic 40 and ProDesign Logic 50.01; Group 3: re-instrumentation with Gates-Glidden drills and manual instruments K-file and Hedstroem files. For filling material removal analysis, the samples were scanned in a microtomograph device SkyScan 1174, for comparison of images taken before and after removing the root canal filling, and in each sample the volume was calculated at four levels (apical 1, apical 2, middle and cervical). These samples were split and analyzed by scanning electron microscopy to visualize filling material residues before and after irrigant activation with an ultrasonic device and with the EasyClean system used in continuous rotary motion. Statistical analyses were performed using Kruskal-Wallis, Friedman, Wilcoxon and Dunn tests. Furthermore, the minimum inhibitory concentration of an irrigating solution containing silver nanoparticles was determined against strains of Enterococcus faecalis, by using the microdilution method. Additionally, the antimicrobial activity of silver nanoparticle solution, 2% chlorhexidine and 2.5% sodium hypochlorite was tested against Enterococcus faecalis biofilm in vitro. For biofilm formation, bovine dentin blocks were placed in 24-well culture plates and Enterococcus faecalis biofilm was developed for 21 days. The dentin blocks were divided into 9 experimental groups of 5 blocks each, according to the irrigating solution used and the time in contact with the irrigant (5, 15 and 30 minutes). The samples were stained with Live/Dead reagent for analysis by confocal laser scanning microscopy (CLSM). Finally, the antimicrobial action of these solutions was tested after dentinal tubules were contaminated with Enterococcus faecalis. Dentin tubes were made from bovine incisors, taken to a centrifuge and infected with Enterococcus faecalis. The dentin tubes were treated with silver nanoparticle solution, 2% chlorhexidine and 2.5% sodium hypochlorite, and analyzed by CLSM to assess the antimicrobial activity of these solutions against bacteria in the dentinal tubules. The results showed that residues of filling material were found after root canal reinstrumentation in all groups. No significant difference was observed in removal of filling material between the reciprocating instruments Reciproc and ProDesign R and between rotary instruments Mtwo 40 and ProDesign Logic 40. The ProDesign Logic 50/.01 instrument significantly improved the removal of filling material compared with the use of Reciproc and ProDesign R instruments. The apical levels presented greater amount of remnant filling material compared with middle and cervical levels. After canal reinstrumentation, the passive ultrasonic irrigation and irrigant agitation with EasyClean significantly improved the removal of residual filling material in all root canal thirds. There was no significant difference between the performance of ultrasonics and EasyClean regarding the removal of residual filling material, as well no significant difference was observed in the removal of these residues when comparing apical, middle and cervical thirds. Regarding the action of the irrigating solutions against Enterococcus faecalis, the minimum inhibitory concentration of silver nanoparticle solution capable of eliminating this microorganism in broth and agar plates was 94 ppm. After the irrigation of Enterococcus faecalis biofilm, the silver nanoparticle solution was significantly less effective in killing bacteria compared with chlorhexidine when used for time of contact of 5 minutes. The sodium hypochlorite solution presented antimicrobial activity significantly higher compared with the silver nanoparticle solution and chlorhexidine. This solution also presented higher ability to dissolve biofilm in all times tested, whereas the silver nanoparticle solution presented higher ability to dissolve biofilm compared with chlorhexidine in times of 5 and 15 minutes. In infected intratubular dentin with Enterococcus faecalis, the sodium hypochlorite solution presented significant higher effectiveness than the silver nanoparticle solutions and chlorhexidine, especially in middle third and deep areas of the root canal. When comparing the antimicrobial activity of these solutions in biofilm and infected intratubular dentin, it was shown that when the silver nanoparticle solution was used for shorter periods of time, it was more effective in intratubular dentin compared with biofilm. On the other hand, with longer time of 30 minutes, the number of viable bacteria was higher in intratubular dentin than in biofilm, which was also observed when using the sodium hypochlorite solution in this time of action. It was concluded that to increase the rate of success in endodontic retreatment, the combination of the use of reciprocating and rotary instruments in the removal of filling material, the agitation of irrigants and the use of antimicrobial agents could be used in an attempt to eliminate bacteria that resisted to endodontic treatment. Em casos de insucesso do tratamento endodôntico, uma alternativa seria o retratamento do canal radicular. Durante este procedimento deve haver remoção de todo o material obturador para que seja realizada novo preparo biomecânico e nova obturação do sistema de canais radiculares. Bactérias são o principal fator etiológico em casos de fracasso da terapia endodôntica, e esta infecção persistente pode estar relacionada à capacidade dos microganismos em penetrar nos túbulos dentinários. Por este motivo é necessário o uso de soluções na irrigação do canal radicular com ação antimicrobiana e com boa tolerância tecidual. O objetivo deste trabalho foi avaliar o desempenho dos instrumentos de diferentes ligas metálicas na desobturação do canal radicular durante o retratamento endodôntico de incisivos laterais superiores com curvatura apical, por meio da microtomografia computadorizada e análise destas amostras no microscópio eletrônico de varredura após a ativação de irrigantes. Posteriormente foi avaliada a capacidade antimicrobiana de um irrigante do canal radicular contendo nanopartículas de prata, clorexidina a 2% e hipoclorito de sódio a 2,5% frente ao biofilme de Enterococcus faecalis e à dentina contaminada com este mesmo microrganismo. Foram selecionados trinta incisivos laterais superiores humanos extraídos que apresentavam curvatura apical. Os dentes foram instrumentados, obturados e divididos em três diferentes grupos de acordo com o protocolo de remoção do material obturador do canal radicular: no Grupo 1: a desobturação foi realizada com os instrumentos Reciproc R25, Mtwo 40 e ProDesign Logic 50.01; no Grupo 2: foram utilizados os instrumentos ProDesign R, ProDesign Logic 40 e ProDesign Logic 50.01; e no Grupo 3: a desobturação foi realizada com brocas de Gates-Glidden e instrumentos manuais tipo K e Hedstroem. Para a análise da remoção do material obturador, as amostras foram escaneadas em micrótomogafo SkyScan 1174 para que fossem comparadas as imagens antes e após a desobturação do canal radicular, e em cada amostra este volume foi calculado nos quatro níveis (apical 1, 2, médio e cervical). Estas amostras posteriormente foram clivadas e analisadas no microscópio eletrônico de varredura, para a visualização de resíduos de material obturador antes e após a ativação de irrigantes com o ultrassom e com o sistema EasyClean utilizado em rotação contínua. Para a análise estatística dos resultados foram utilizados os testes de Kruskal-Wallis, Friedman, Wilcoxon e Dunn. Posteriormente, foi determinada a concentração inibitória mínima de uma solução irrigadora do canal contendo nanopartículas de prata frente a cepas de Enterococcus faecalis, através do método de diluição em caldo. Em seguida, foi testada a atividade antimicrobiana das soluções de nanopartículas de prata, da clorexidina a 2% e do hipoclorito de sódio a 2,5% sobre o biofilme de Enterococcus faecalis in vitro. Para isso, foram utilizados blocos de dentina bovina colocados em placas de 24 poços e biofilme de Enterococcus faecalis foi formado durante 21 dias. Os blocos de dentina foram divididos em 9 grupos experimentais com 5 blocos cada um, em função dos irrigantes avaliados e do tempo de exposição à solução irrigadora (5, 10 e 15 minutos). As amostras foram coradas com corante Live/Dead para posterior análise no microscópio confocal de varredura a laser (MCVL). Por fim, foi testada a atividade antimicrobiana destas soluções irrigadoras após a contaminação de túbulos dentinários com Enterococcus faecalis. Foram confeccionados tubos de dentina a partir de incisivos bovinos que foram levados à centrífuga e contaminados com Enterococcus faecalis. Os tubos de dentina receberam tratamento com a solução de nanopartículas de prata, com clorexidina a 2% e com hipoclorito de sódio a 2,5%, e foram analisados no MCVL para avaliar a atividade antimicrobiana das soluções sobre bactérias presentes nos túbulos dentinários. Os resultados demonstraram que resíduos de material obturador foram encontrados após a desobturação do canal em todos os grupos. Não houve diferença significante na remoção de material obturador entre os instrumentos reciprocantes Reciproc e ProDesign R e entre os instrumentos rotatórios Mtwo 40 e ProDesign Logic 40. O instrumento ProDesign Logic 50/.01 melhorou significantemente a remoção de material obturador comparado com o uso dos instrumentos Reciproc e ProDesign R. Os níveis apicais apresentaram uma maior quantidade de material obturador remanescente comparados com os níveis médio e cervical. Após a desobturação do canal radicular, a irrigação ultrassônica passiva e agitação dos irrigantes com o EasyClean melhoraram significantemente a remoção de resíduos de material obturador em todos os terços do canal radicular. Não houve diferença significante no desempenho do ultrassom e do EasyClean em relação à remoção de resíduos de material obturador, assim como não foi observada diferença significante na remoção destes resíduos quando comparados terços apical, médio e cervical. No que diz respeito à ação das soluções irrigadoras sobre o Enterococcus faecalis, a concentração inibitória mínima da solução de nanopartículas de prata capaz de eliminar este microrganimo em meio de cultura e ágar foi de 94 ppm. Após a irrigação no biofilme de Enterococcus faecalis, a solução de nanopartículas de prata foi significantemente menos efetiva em matar bactérias comparada com a clorexidina quando utilizadas pelo tempo de 5 minutos. A solução de hipoclorito de sódio apresentou atividade antimicrobiana significantemente maior comparada com as soluções de nanopartículas de prata e clorexidina. Essa solução ainda apresentou maior capacidade de dissolução do biofilme em todos os tempos testados, enquanto que a solução de nanopartículas de prata apresentou maior capacidade de dissolver o biofilme comparada à clorexidina nos tempos de 5 e 15 minutos. Na dentina intratubular infectada com Enterococcus faecalis, a solução de hipoclorito de sódio apresentou efetividade significantemente maior que as soluções de nanopartículas de prata e clorexidina, principalmente no terço médio e na região profunda do canal radicular. Quando comparada a atividade antimicrobiana destas soluções no biofilme e na dentina intratubular infectada, verificou-se que quando a solução de nanopartículas de prata foi utilizada por um tempo mais curto, foi mais efetiva na dentina intratubular comparada com o biofilme. De maneira contrária, com tempo maior de 30 minutos, o número e bactérias viáveis foi maior na dentina intratubular do que no biofilme, o que ocorreu também com a solução de hipoclorito de sódio neste tempo de ação. Conclui-se que para se aumentar a chance de sucesso em tratamentos retratamentos endodônticos, a combinação do uso de instrumentos reciprocantes e rotatórios na desobturação do canal, agitação de irrigantes e uso de agentes com capacidade antimicrobiana podem ser utilizados na tentativa de se eliminar bactérias resistente ao tratamento endodôntico. OBJECTIVES: The aims of this study were to quantify tug-back by measuring the pulling force and investigate the correlation of clinical tug-back pulling force with in vitro gutta-percha (GP) cone adaptation score using micro-computed tomography (µCT). MATERIALS AND METHODS: Twenty-eight roots from human single-rooted teeth were divided into 2 groups. In the ProTaper Next (PTN) group, root canals were prepared with PTN, and in the ProFile (PF) group, root canals were prepared using PF (n = 14). The degree of tug-back was scored after selecting taper-matched GP cones. A novel method using a spring balance was designed to quantify the tug-back by measuring the pulling force. The correlation between tug-back scores, pulling force, and percentage of the gutta-percha occupied area (pGPOA) within apical 3 mm was investigated using µCT. The data were analyzed using Pearson's correlation analysis, one-way analysis of variance (ANOVA) and Tukey's test. RESULTS: Specimens with a strong tug-back had a mean pulling force of 1.24 N (range, 0.15–1.70 N). This study showed a positive correlation between tug-back score, pulling force, and pGPOA. However, there was no significant difference in these factors between the PTN and PF groups. Regardless of the groups, pGPOA and pulling force were significantly higher in the specimens with a higher tug-back score (p < 0.05). CONCLUSIONS: The degree of subjective tug-back was a definitive determinant for master cone adaptation in the root canal. The use of the tug-back scoring system and pulling force allows the interpretation of subjective tug-back in a more objective and quantitative manner.

Abstract The relationship of the main foramen to the anatomic root apex has been the subject of several studies. Although they are anatomically close, they rarely coincide, and their distance can vary according to age or tooth type, ranging from 0.2 to 3.0 mm. The aim of this short communication was to evaluate the distance between the main foramen of independent middle mesial canals (MMCs) and the anatomical mesial root apex of mandibular first molars using the micro-computed tomography. Twenty-five mandibular first molars with MMCs were scanned (resolution of 9.9 µm), and the distance from its main foramen to the anatomical apex was evaluated. Overall, the distance ranged from 0.2 to 2.4 mm; however, in 3 specimens the distance was greater than 3 mm. This report demonstrates that the exit of the main foramen of the MMC varies considerably and could approach a substantial distance from the anatomical apex greater than previously reported in the literature.


Resumo A relação do forame principal com o ápice anatômico da raiz tem sido objeto de vários estudos. Embora estejam anatomicamente próximos, eles raramente coincidem, e a sua distância pode variar de acordo com a idade ou o tipo de dente, oscilando entre 0,2 e 3,0 mm. O objetivo deste artigo é relatar que a saída foraminal do canal mediano (MMC), presente na raiz mesial de molares inferiores, pode apresentar variações significativas, atingindo distâncias acima de 3 mm do ápice anatômico. Vinte e cinco primeiros molares inferiores com MMC foram escaneados (resolução de 9,9 mm) e a distância do forame principal ao ápice anatômico avaliada. Em geral, a distância mostrou variações consideráveis; no entanto, em 3 espécimes esta distância ficou acima de 3 mm. Este estudo teve como objetivo relatar que a saída do forame principal de canais medianos de primeiros molares inferiores pode apresentar variações consideráveis, podendo atingir distâncias maiores do que as relatadas previamente na literatura.

ABSTRACT

OBJECTIVES: The aim of this study was to compare the osteogenic effects of demineralized dentin matrix (DDM) combined with recombinant human bone morphogenetic protein-2 (rhBMP-2) in rabbit calvarial defects with DDM and anorganic bovine bone (ABB) combined with rhBMP-2. MATERIALS AND METHODS: Four round defects with 8-mm diameters were created in each rabbit calvaria. Each defect was treated with one of the following: 1) DDM, 2) ABB/rhBMP-2, or 3) DDM/rhBMP-2. The rhBMP-2 was combined with DDM and ABB according to a stepwise dry and dip lyophilizing protocol. Histological and microcomputed tomography (µCT) analyses were performed to measure the amount of bone formation and bone volume after 2- and 8-week healing intervals. RESULTS: Upon histological observation at two weeks, the DDM and ABB/rhBMP-2 groups showed osteoconductive bone formation, while the DDM/rhBMP-2 group showed osteoconductive and osteoinductive bone formation. New bone formation was higher in DDM/rhBMP-2, DDM and ABB decreasing order. The amounts of bone formation were very similar at two weeks; however, at eight weeks, the DDM/rhBMP-2 group showed a two-fold greater amount of bone formation compared to the DDM and ABB/rhBMP-2 groups. The µCT analysis showed markedly increased bone volume in the DDM/rhBMP-2 group at eight weeks compared with that of the DDM group. Notably, there was a slight decrease in bone volume in the ABB/rhBMP-2 group at eight weeks. There were no significant differences among the DDM, ABB/rhBMP-2, and DDM/rhBMP-2 groups at two or eight weeks. CONCLUSION: Within the limitations of this study, DDM appears to be a suitable carrier for rhBMP-2 in orthotopic sites.

ABSTRACT

We report on a case of potential envenomation caused by multiple bites by the aglyphous opisthodont snake Leioheterodon madagascariensis in the left thumb of a healthy adult man, which is among the most serious snakebites hitherto reported from Madagascar. The adult snake (total length > 1 meter) was unusually aggressive before and during capture. The symptoms included extensive bleeding, severe local pain, and substantial swelling of the hand and the distal part of the lower arm. The swelling disappeared entirely after five days, but pain in the thumb (when moved) was recognizable even longer. Although L. madagascariensis is widespread and common in anthropogenic habitats in eastern and western Madagascar, this case report seems to be the first description of long-lasting symptoms of its bite. Since aglyphous snakes are relatively rarely involved in "envenomation" and because hemolytic activity has been recorded in the secretions of the Duvernoy's glands of Leioheterodon, we describe its dentition using microcomputed tomography and discuss the potential mode of envenomation in this case.(AU)

ABSTRACT

PURPOSE: The purpose of this study was to evaluate bone formation around recombinant human bone morphogenetic protein (rhBMP-2)-coated implants placed with or without absorbable collagen sponge (ACS) in rabbit maxillary sinuses. METHODS: The Schneiderian membrane was elevated and an implant was placed in 24 sinuses in 12 rabbits. The space created beneath the elevated membrane was filled with either blood (n=6) or ACS (n=6). In the rabbits in which this space was filled with blood, rhBMP-2-coated and non-coated implants were alternately placed on different sides. The resulting groups were referred to as the BC and BN groups, respectively. The AC and AN groups were produced in ACS-grafted rabbits in the same manner. Radiographic and histomorphometric analyses were performed after eight weeks of healing. RESULTS: In micro-computed tomography analysis, the total augmented volume and new bone volume were significantly greater in the ACS-grafted sinuses than in the blood-filled sinuses (P<0.05). The histometric analysis showed that the areas of new bone and bone-to-implant contact were significantly larger in the AC group than in the AN group (P<0.05). In contrast, none of the parameters differed significantly between the BC and BN groups. CONCLUSIONS: The results of this pilot study indicate that the insertion of ACS after elevating the Schneiderian membrane, simultaneously with implant placement, can significantly increase the volume of the augmentation. However, in the present study, the rhBMP-2 coating exhibited limited effectiveness in enhancing the quantity and quality of regenerated bone.

ABSTRACT

Panax ginseng, also known as Korean ginseng, has long been used as a broad tonic in Oriental medicine to augment vitality, health, and longevity, particularly in older people. This study investigated the effects of Korean red ginseng (RG) on bone loss in ovariectomized (OVX) mice. C3H/HeN mice (10-weeks-old) were divided into sham and OVX groups. OVX mice were treated with vehicle, 17beta-estradiol (E2), RG (oral administration, 250 mg/kg/day), or RG (intraperitoneal administration, 50 mg/kg/every other day) for 6 weeks. Serum E2 concentration and alkaline phosphatase (ALP) activity were measured. Tibiae were analyzed using microcomputed tomography. Biomechanical properties and osteoclast surface level were measured. There was no significant difference in the degree of grip strength, body weight, uterine weight, mechanical property, tibiae length, or tibiae weight between the OVX and RG-treated groups. Compared with the OVX group, the serum ALP level was significantly lower in the RG-treated groups. Serum E2 levels and osteoclast surface levels did not change between the OVX and RG-treated groups. RG could not preserve trabecular bone volume, trabecular bone number, trabecular separation, trabecular thickness, structure model index, or bone mineral density of the proximal tibiae metaphysic. In conclusion, there was no definite effect of RG on OVX-induced bone loss in C3H/HeN mice.
